Appointments

Please arrive 10-15 minutes prior to appointment to complete paperwork and update information.  Patients that miss appointments without 24 hour notice may be assessed a $25 no-show fee.

Payment is expected at the time of service.  We are not in network with any commercial insurance carriers.  You will be considered out of network by your insurance. As a courtesy, we will electronically submit your paid claim bill to your insurance company for reimbursement to you directly. Occasionally, we will need to mail the appropriate form to you for submission to the insurance from you directly
